Description: We are seeking a dedicated and compassionate Case Manager to join our team at D House of Angels. As a Case Manager, you will be responsible for providing comprehensive support and assistance to individuals and families affected by domestic violence and sexual assault. You will play a crucial role in helping clients navigate various resources, access services, and develop personalized action plans to achieve their goals.
Responsibilities:
- Conduct intake assessments and develop individualized service plans based on client needs and goals.
- Provide crisis intervention and emotional support to clients in a compassionate and nonjudgmental manner.
- Coordinate and facilitate access to resources such as housing, legal assistance, healthcare, and counseling services.
- Advocate for clients' rights and assist with navigating systems such as social services, law enforcement, and legal proceedings.
- Monitor client progress and provide ongoing support, guidance, and advocacy as needed.
- Maintain accurate and up-to-date client records and documentation in accordance with agency policies and procedures.
- Collaborate with community partners and service providers to ensure comprehensive and coordinated care for clients.
- Participate in case conferences, team meetings, and trainings to enhance knowledge and skills in supporting survivors of domestic violence and sexual assault.
Qualifications:
- Bachelor's degree in social work, psychology, counseling, or related field (Master's degree preferred).
- Previous experience working with survivors of domestic violence, sexual assault, or trauma is highly desirable.
- Strong interpersonal and communication skills with the ability to establish rapport and build trust with clients.
- Knowledge of community resources and services available to survivors of domestic violence and sexual assault.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Commitment to trauma-informed care, cultural sensitivity, and confidentiality.
- Proficiency in maintaining accurate and detailed documentation.
