Case Manager

Description: We are seeking a dedicated and compassionate Case Manager to join our team at D House of Angels. As a Case Manager, you will be responsible for providing comprehensive support and assistance to individuals and families affected by domestic violence and sexual assault. You will play a crucial role in helping clients navigate various resources, access services, and develop personalized action plans to achieve their goals.

Responsibilities:

  1. Conduct intake assessments and develop individualized service plans based on client needs and goals.
  2. Provide crisis intervention and emotional support to clients in a compassionate and nonjudgmental manner.
  3. Coordinate and facilitate access to resources such as housing, legal assistance, healthcare, and counseling services.
  4. Advocate for clients' rights and assist with navigating systems such as social services, law enforcement, and legal proceedings.
  5. Monitor client progress and provide ongoing support, guidance, and advocacy as needed.
  6. Maintain accurate and up-to-date client records and documentation in accordance with agency policies and procedures.
  7. Collaborate with community partners and service providers to ensure comprehensive and coordinated care for clients.
  8. Participate in case conferences, team meetings, and trainings to enhance knowledge and skills in supporting survivors of domestic violence and sexual assault.

Qualifications:

  • Bachelor's degree in social work, psychology, counseling, or related field (Master's degree preferred).
  • Previous experience working with survivors of domestic violence, sexual assault, or trauma is highly desirable.
  • Strong interpersonal and communication skills with the ability to establish rapport and build trust with clients.
  • Knowledge of community resources and services available to survivors of domestic violence and sexual assault.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
  • Commitment to trauma-informed care, cultural sensitivity, and confidentiality.
  • Proficiency in maintaining accurate and detailed documentation.
